What's the format for submitting a typed-up Cover Letter into an online job application?

If there's no option to attach a cover letter document, but instead they want you to type up a Cover Letter into a blank space, how should it be done?

Should/do I still include a header (my name and address; contact info), date, and inside address of the company I'm writing to?

What changes? How much space should I keep between each thing?

Use copy paste. Copy the Cover letter from wherever you have it and paste it into the blank space. Don't leave out anything
