Should I include multiple positions with same company on resume?

I have worked for the same company for 12 years. I started out as a customer service associate, was promoted to Quality Assurance Manager in 2005, and Store Manager in 2008. Should I list each position separately with the dates/duties/responsibilities for each one or should I just include the most recent position?

If it's just the most recent, should I put as a starting date the date I started with the company or the date I was promoted to the most recent position?

There is nothing wrong with showing significant changes of job role within the company, especially if they imply promotion. Far better than just doing the same job for 12 years.

Yes, list them all, the dates, duties and your responsibilities etc.
