Could use a form I can download for free and print out to apply for any job. Anyone know where I can find a standard application form?

Unfortunately this is no way to get a job, your answers will be generic and specific only to you and not the person recruiting. This is why you see some adverts requesting application forms and stating no CV's please.
If you are looking for a template try looking for a CV in word

I don't believe that there is a standard job application form. The norm seems to be a CV and a written job application, though, nowadays, applications are increasingly wanted on-line. Have a look here: http://www.edinburgh.gov.uk/cec/jobs/applicationForms/StandardApplicationForm.pdf
Maybe this link will give you an idea of how a "standard" job application form might look like?

You can try again, and they will likely take the other money into account. But as you are the main sponsor, and from a country that has a rather unfortunate history of illegal immigration, it's going to be tough. Also, it looks like from a statement in your denial letter that there was another issue besides your bank balance, namely "Furthermore, you have not shown your own personal financial circumstances." Now I'm not sure exactly what this means, but I'm thinking this means you didn't supply pay stubs, employment contract, housing lease, bills-in-your-name and/or other documents they would need to verify that you really can keep yourself in a stable situation, much less keep someone else in a stable situation. I think you need to address this even more importantly than the pure funding issue. Good luck.
