1. Do employers contact references before they call you for an interview?
2. Where do I write this on a resume? Do I just make a references heading and then write this underneath?

usually, references are placed at the end of the resume.

Personally, I think you don't need to say anything about references. I wouldn't bother calling your references until I'm actually considering you seriously, and it is assumed that you WILL provide references upon request. Therefore, save the space for something else that will sell you to the hiring manager.
However, if you really want to include it, then all of it is at the end of the resume. "References upon request" is the last line of your resume. Actually references would also be at the end with a category of "References" followed by name, relationship, contact info.

Usually they call after the interview to make sure your credentials match what you told them during the interview process and application.
I usually provide them with the references with the resume on the 2nd page. I make sure to have 3 professional references (work related or anyone you worked with) and 3 personal (volunteer group, those that know you but have not worked with you that can vouch for your personality). It's good to have the reference's title and business with all of their contact information as this will give you more credibility. Just remember to get permission from them before putting them as reference and after you apply for a job, let the references know what kind of positions you applied for and what they are looking for in an employee. Referees are placed at the bottom of your resume once you have written everything. Depends on the Job you are applying for and the organization if your references are contacted generally they interview you first and if they are unsure or its competitive they may have short listed a few people and then may call upon a references to determine who gets the job
